Assist-Life Stage / Transition

Being an NDIS licenced provider, we are aware of the various obstacles that people face at various phases of their lives. Our goal is to enable people with disabilities to live fulfilling lives by providing them with specialised services and programmes that are designed to meet their individual needs.

At Australian Care Centre, we understand how important it is to attend to people's various needs as they move through important life transitions. Our Assist-Life Stage / Transition Programme has been carefully crafted to provide tailored assistance and direction to people with disabilities as they approach important turning points in their lives, such as leaving school to engage in after-school activities, starting a career, or moving into independent living. We are dedicated to making sure that every participant gets the support they need to effectively complete these transitions and realise their goals.
